The UniCare Fit plan is a PPO plan that includes unlimited office visits for only $30. Deductibles from $500-$5000. Preventative benefits, that can include x-rays, pap smear, PSA screening and mammograms. In and out patient hospitalazation. Physical, Occupational and Speech Therapy, Acupuncture and Chiropractic care. Prescription coverage and last, but certanily not least affordable premiums.
